The Process

1. First Inspection
   • Initial site inspection; conducted prior to approval of application
2. Subgrade (if required)
   • A site preparation inspection may be required if system is to be built on clay soils or bedrock
   • Fee of $178 (if conducted by OSSO)
3. Second Inspection
   • Once system is built with components in place and prior to backfill, an inspector must visit the site
   • The following supporting documents are required upon submission of inspection request:
     • As-built drawing and components
     • Engineer’s report (if designed by Engineer)
     • Grain size analysis and weigh bills
4. Third Inspection
   • Prior to issuance of Certificate, final inspection is required
   • Inspector to ensure that all components are in full working order and backfill has been properly graded

Submission Requirements

1. First Inspection
   • No forms required - part of formal review process
2. Subgrade (if required)
   • Completed inspection request form indicating ‘Subgrade’
3. Second Inspection
   • Completed inspection request form indicating ‘Installation Inspection’
   • Supporting documents, including:
     • As-built drawings/components
     • Engineer’s report (if Engineered)
     • Grain size analysis and weigh bills
4. Third Inspection
   • Completed inspection request form indicating ‘Final Grade Inspection’
5. Fee payment at time of submission (if required)
